It seems that since I started the quest before the patch became live, I am still able to get the second vial and acquire the Hand of A'dal title. For a time, anyway...

Soooooo, if anyone has the Vials of Eternity quest and needs Kael or Vashj kills, and if anyone else wants to help out their stabby gnome, can we get a group together sometime on an off-raid night?

You might know this, I didn't when I got my first title.....
You have to "select" your title for it to be seen. "C" to bring up character screen and then at the top there should be a pull down menu of any available titles you have.
Hopefully this helps.

You must be missing some other part like the BT attunement perhaps? I saw someone get the Hand of Ad'al achievement in guild chat post-patch. Can't remember who though.

unless blue post on forum is wrong, you can get title if you had quest in log for patch 3.0, if not.....no title
